Salivary Metabolomics in the Differential Diagnosis of Mumps and Acute Suppurative Parotitis: a Preliminary Report

唾液代谢组学在流行性腮腺炎和急性化脓性腮腺炎中的初步分析

Abstract:

Objective: Salivary metabolomics was used to screen potential biomarkers for early prediction of mumps and acute suppurative parotitis. Methods: 18 patients of mumps and 18 patients of acute suppurative parotitis treated in the department from June 2017 to March 2019 were selected. The saliva metabolomics method based on ultra performance liquid chromatography tandem quadrupole time-of-flight mass spectrometric method (UHPLC-QTOF/MS) was used to analyze the differential metabolites of the two groups. Results: 32 different salivary metabolites were screened out in the detection cluster (8 patients with mumps and 8 patients with acute suppurative parotitis). Further evaluate the feasibility of these potential biomarkers in differentiating between mumps and acute suppurative parotitis in the validation cluster (10 patients with mumps and 10 patients with acute suppurative parotitis). Two metabolites 【undecanoic acid and phosphatidyl cholines(PC)】 (P-18:1/18:1) were screened out and the area under the receiver operating characteristic curve (ROC) was 0.926. Conclusion: The combination biomarker of undecanoic acid and PC(P-18:1/18:1) selected by salivary metabolomics may be a good "classifier" for patients with acute suppurative parotitis and mumps and may provide a new method for early differential diagnosis.

目的: 应用唾液代谢组学方法筛选并鉴别诊断早期流行性腮腺炎和急性化脓性腮腺炎的潜在生物标志物。方法: 选取 2017-06—2019-03就诊于上海市第五人民医院口腔科的流行性腮腺炎患者和急性化脓性腮腺炎患者各18例作为研究对象,使用基于超高效液相色谱-四级杆飞行时间高分辨质谱联用技术(ultra performance liquid chromatography tandem quadrupole time-of-flight mass spectrometric method,UHPLC-QTOF/MS)的唾液代谢组学方法对2组样本进行差异代谢物分析。结果: 在发现集(流行性腮腺炎和急性化脓性腮腺炎样本各8例)中筛选出32个差异代谢物。在验证集(流行性腮腺炎和急性化脓性腮腺炎样本各10例)中评估这些差异代谢物区分流行性腮腺炎和急性化脓性腮腺炎的可行性,筛选出标志物组合:十一酸和磷脂酰胆碱(P-18:1/18:1),其接受者操作特征曲线(receiver operating characteristic curve, ROC)下面积为0.926。结论: 该标志物组合可能成为急性化脓性腮腺炎与流行性腮腺炎很好的“分类器”,可能为早期诊断流行性腮腺炎和急性化脓性腮腺炎提供一种新的手段。
