锥形束CT在上颌埋伏尖牙正畸牵引手术开窗定位中的应用

CBCT in the Surgical Exposure and Orthodontic Management of Impacted Maxillary Canines

摘要: 目的:评估锥体束CT在上颌埋伏尖牙正畸牵引手术开窗定位中的作用。方法:对12颗上颌埋伏尖牙拍摄锥体束CT,观察其解剖形态、空间位置及毗邻关系,行外科开窗手术后配合正畸牵引,引导其萌出到生理位置。结果:锥体束CT清楚地显示了埋伏上颌尖牙的解剖形态、空间位置及毗邻关系。开窗手术术中所见与锥体束CT显示的上颌埋伏尖牙空间位置信息相一致。12颗埋伏上颌尖牙经外科开窗手术后成功牵引萌出,尖牙形态良好,牙髓活力正常,牵引未引起邻牙牙根吸收。结论:锥体束CT在埋伏上颌尖牙的定位诊断中具有一定的应用价值。

Abstract: Objective: To review the application of dental cone beam CT for positioning in surgical exposure combined orthodontic traction of impacted maxillary canines. Methods: 12 pediatric impacted maxillary canines (age 10-13 years) were included in this review. The morphology, location and their relationship with adjacent anatomical structures of the impacted maxillary canines were examined by CBCT. Based on the clinical presentation, radiographic examination, and laboratory investigations, an interdisciplinary approach between the oral radiologist, oral surgeon, orthodontist were involved. Surgical exposure of the impacted maxillary canines followed by guided eruption through orthodontic therapy was planned. Results: The dental cone beam CT images provided a much more accurate display of the morphology, location and their relationship with adjacent anatomical structures of the impacted canines. 12 impacted maxillary canines were brought into occlusion successfully with normal morphology, vital dental pulp and non resorption of adjacent tooth. Conclusion: Dental cone beam CT is valuable for precise positioning of impacted maxillary canine.
