两种预防性抗生素使用方法在正颌手术中的应用比较

Perioperative Antibiotic Prophylaxis in Orthognathic Surgery: a Clinical Trial

摘要: 目的:通过回顾分析我院正颌手术患者预防性使用抗生素两种方法的临床效果,探讨正颌手术预防性使用抗生素的有效方法。方法:回顾性分析我院2015?鄄04—2016?鄄04期间,入院进行正颌手术治疗的60位择期手术患者,按入院时间及预防性应用抗生素的方法将患者分为2组。A组30例,为我院规范化使用抗生素前(2015?鄄10前)收治的患者,术前未使用抗生素,术后静脉应用一代头孢菌素7 d至出院。B组30例,为我院规范化使用抗生素后(2015?鄄10后)收治的正颌手术患者,术前30 min静滴一代头孢菌素;术后每8 h使用1次抗生素,直至术后48 h停药。对两组患者体温、伤口脓性分泌、疼痛、肿胀、伤口愈合情况的差异进行比较。结果:两组患者均无术后感染发生。结论:正颌手术围手术期预防性使用抗生素,首剂药物术前30 min给予,术后48 h停药,是安全并有效可行的。

Abstract: Objective: The purpose of this study was to evaluate the efficacy of antibiotic prophylaxis in orthognathic surgery between different methods and conclude a proper one. Methods: To retrospectively analyze the infection results of selected 60 patients in orthognathic surgery, from April 2015 to April 2016. According to the hospitalization date and the application of antibiotics, the patients were divided into 2 groups. In group A, the period of hospitalization were from April 2015 to October 2015, and the postoperative antibiotics application lasted for 7 days until discharged. In group B, the period of hospitalization were from October 2015 to April 2016, and each patient intravenously received a single-dose of first generation cephalosporin 30 minutes before surgery, and one dose of antibiotics administered every 8 hours until 48 hours postoperation. Patients’ temperature, purulent drainage, pain or tenderness, local swelling, and wound healing were recorded and compared. Results: There was no postoperative infection happened in both groups. Conclusion: It is suggested that the first dose antibiotics given 30 mins before surgery and continued for two days is more economic, safe and effective.
