钛种植体折裂的临床处理及相关因素讨论

Case Report of Dental Implant Fracture and Related Factors Analysis

摘要: 目的:通过回顾1例高强度钛种植体折裂病例,结合文献探讨种植体折裂的相关影响因素及临床处理方法。方法:患者左上后牙缺失多年,3年前行种植修复。种植体负载2年后折裂,要求重新种植修复。经完善临床及影像学检查,择期手术取出种植体6个月后重新种植修复。结果:种植体取出6个月后复查,缺牙区软组织恢复良好,角化龈充足,重新植入种植体4个月后复查骨结合良好。结论:种植体折断是临床较为少见的严重的机械并发症,一般与修复体非被动就位、修复体负荷过大、种植位点设计、种植体金属疲劳等有关,对已发生折裂的种植体应考虑完全取出。种植术前应根据患者情况科学考虑种植、修复设计,以防止种植体折裂的出现。

Abstract: Objective: To investigate the effects of the related factors and clinical treatments on the implant fracture with a typical case. Methods: A patient with missing of left maxillary molar for several years, required an implant restoration 3 years ago. The implant was fractured 2 years after loading, then the patient required an implant re-placement. The fractured implant was removed after thorough clinical and radiological examination, and implant re-placement was performed in 6 months. Results: 6 months after removing the fractured implant, the keratinized gingiva was sufficient in the edentulous area. The implant showed excellent osseointegration 4 months after the re-placement. Conclusion: The fracture of dental implants is a rare and serious occurrence in clinical settings, which is generally related to non-passive fitting of the prosthetic crown, overloading, position of the implant, metal fatigue of implant. Complete removal of fractured implants should be considered in clinical treatment. Implant and prosthesis design should be considered scientifically according to the patient's situation before the implant surgery, to prevent the occurrence of implant fracture.
