Biomechanical Analysis of Mini Titanium Plate Internal Fixation in the Treatment of Mandibular Fracture

下颌骨骨折78例临床疗效观察

Abstract: Objective: To explore the effectiveness and safety of mini titanium plate internal fixation for treatment of mandibular fracture. Methods: From February 2013 to January 2016, 78 patients suffered by fracture of the mandible, were selected and randomly divided into observation group and control group with 39 patients in each group. The control group was treated by dental arch splint fixation and intermaxillary traction, the observation group was treated with mini titanium plate and fixation with mini screw, recorded the effectiveness and safety. Results: In the observation group, all patients were healed by first intention, and the fracture were healed well. After the 3 months of treatment, there were 25 patients of normal mouth opening, 13 patients were mild limitation and 1 patient was moderate to severe restriction; while in the control group, there were 18 patients, 12 patients and 9 patients respectively, treatment effects in the observation group was significantly better than the control group (P<0.05). The incidence rates of infection, nerve injury, root injury and other complications in the observation group was 10.3%, while in the control group was 33.3%, the difference was significant (P<0.05). The bone mineral density in the observation group after 1 months and 3 months were (70.11±8.24) g/cm3 and (232.14±44.19) g/cm3 respectively, in the control group were (65.89±9.22) g/cm3 and (191.34±14.29) g/cm3  respectively, also showed significant difference (P<0.05). Conclusion: The present review suggests mini titanium plate internal fixation in the treatment of mandibular fracture can effectively reduction of fractured bone and promote the improvement of bone density, reduce the incidence of postoperative complications, promote the restoration of normal occlusal relationship.

摘要: 目的:探讨微型钛板坚强内固定治疗下颌骨骨折的有效性与安全性。方法:2013-02—2016-01期间,选择在我院诊治的下颌骨骨折患者78例作为研究对象,根据随机数字表法分为观察组与对照组各39例。对照组采用牙弓夹板固定加颌间弹性牵引治疗,观察组采用微型钛板坚强内固定治疗,记录预后有效性与安全性。结果:所有患者手术伤口均Ⅰ期愈合,骨折愈合良好。经过判定,术后3个月观察组的张口度正常25例,轻度受限13例,中重度受限1例;而对照组中张口度正常18例,轻度受限12例,中重度受限9例;观察组效果明显好于对照组(P<0.05)。观察组术后3个月的感染、神经损伤、牙根损伤等并发症发生率为10.3%,对照组为33.3%,观察组明显少于对照组(P<0.05)。观察组术后1个月与术后3个月的骨密度分别为(70.11±8.24) g/cm3和(232.14±44.19) g/cm3,都明显高于对照组的(65.89±9.22)g/cm3和(191.34±14.29) g/cm3,P<0.05。结论:微型钛板坚强内固定治疗下颌骨骨折能促进骨密度的提高,减少术后并发症的发生,促进恢复正常咬合关系,有很好的应用效果。
