Experimental Study on Repairing the Gingival Defects of Mini-pigs with Free Buccal Fat Pad

游离颊脂垫移植修复小型猪牙龈缺损的实验研究

Abstract: Objective: To study the histological changes in the healing wound by repairing the gingival defects with free buccal fat pad in mini-pigs. Methods: Healthy adult female mini-pigs were selected to remove approximately 1 cm×1 cm of buccal gingiva between the lateral incisor and canine bilaterally. The gingiva defects at left side was taken as experimental group and transplanted with a free buccal fat pad, while the gingiva defects at right side set were the control group and treated with a gelatin sponge. Surgical wound in both groups were closed with purse-string suture. At the 3rd days, the 1st, 2nd and 4th weeks after surgery, wound healing of both groups were recorded. Calculating the wound area with Image J and doing statistical analysis with SPSS22. Making a paraffin section for hematoxylin and eosin (HE) staining, then the histological changes during healing was observed under the microscope. Results: At each time point, the wound in experimental group healed better than that in control group. The results of HE staining showed that the epithelial coverage in the experimental group are faster than control group at the 3rd days and 1st week after surgery, and the inflammatory cells within the connective tissue was less in experimental group than in control group. The level of keratinization was higher in the experimental group than in the control group at the 2nd and 4th weeks after surgery. Conclusions: Buccal fat pad can improve the healing of gingival defects, attenuate the inflammation as well as facilitate the epithelial keratinization. It could be used as a supply region for gingival tissue augmentation.

摘要: 目的:研究游离颊脂垫修复小型猪牙龈缺损愈合过程中的组织学变化。方法:选取健康成年雌性小型猪,于双侧侧切牙与尖牙间唇侧切除约1 cm×1 cm大小的牙龈,左侧实验组移植游离颊脂垫,右侧对照组采用明胶海绵止血。术后3 d和1、2、4周拍照记录两组创口愈合情况,使用Image J对伤口面积进行计算,SPSS 22做统计学处理,制作石蜡切片行苏木素-伊红染色(hematoxylin and eosin, HE)染色后在光学显微镜下观察。结果:相同时间点实验组较对照组牙龈缺损创口愈合更佳。HE染色结果显示,术后3 d和术后1周实验组创面较对照组上皮覆盖更快,结缔组织中炎性细胞数更少,术后2周和4周实验组创面新生上皮较对照组先发生上皮角化,且角化程度高。结论:颊脂垫修复可缩短牙龈缺损的愈合时间,减轻炎症,促进上皮角化,是牙龈组织增量的理想供区。
