Therapeutic Effect of Low Temperature Plasma UPPP Combined with Continuous Positive Pressure Ventilation on Moderate and Severe OSAHS

低温等离子UPPP联合持续正压通气治疗中重度OSAHS的疗效分析

Abstract: Objective: To explore the value of low temperature plasma uvulopalatopharyngoplasty (UPPP) combined with continuous positive pressure ventilation in the treatment of moderate and severe obstructive sleep apnea-hypopnea syndrome (OSAHS). Method: 72 patients with moderate and severe OSAHS in our hospital (February 2017-January 2019) were randomly divided into control group (n=36) and research group (n=36). The control group was treated with continuous positive pressure ventilation and UPPP, while the study group was treated with continuous positive pressure ventilation and low temperature plasma UPPP. Sleep breathing indicators [longest sleep apnea duration, SpO2, sleep disorder index (Apnea-Hypopnea Index, AHI)], clinical efficacy, cardio-cerebrovascular events (ischemic cerebrovascular disease, incidence of sinus arrest and arrhythmia) were analyzed before and 12 weeks after treatment. Result: ①Sleep breathing indicators: after 12 weeks of treatment, the longest sleep apnea duration and AHI of the two groups decreased, while SpO2 of both group increased, however the longest sleep apnea duration and AHI of the study group were lower than those of the control group, and SpO2 was higher than that of the control group (P<0.05). ②Clinical efficacy: The total effective rate of the study group (94.44%) was higher than that of the control group (77.78%),P<0.05. ③Cardiovascular and cerebrovascular events: The incidence of cardiovascular and cerebrovascular events in the study group (5.56%) was lower than that in the control group(22.22%),P<0.05. Conclusion: The combination of low temperature plasma UPPP and continuous positive pressure ventilation for the treatment of moderate and severe OSAHS patients can effectively improve the duration of sleep apnea and sleep respiratory disorder index. It is conducive to improving the overall treatment effect of the disease, and can reduce the risk of cardiovascular and cerebrovascular events, therefore has a positive significance in improving the prognosis.

摘要: 目的:探讨低温等离子悬雍垂腭咽成形术(UPPP)联合持续正压通气治疗中重度阻塞性睡眠呼吸暂停低通综合征(OSAHS)的应用价值。方法:选取我院OSAHS中重度患者72例(2017-02—2019-01),随机数字表法分为对照组(n=36)与研究组(n=36)。对照组采取持续正压通气及UPPP治疗,研究组采取持续正压通气及低温等离子UPPP。统计2组治疗前及治疗12周后睡眠呼吸指标状况[最长睡眠呼吸暂停时长、血氧饱和度(SpO2)、睡眠呼吸紊乱指数(AHI)]、临床疗效、心脑血管事件(缺血性脑血管病、窦性停搏、心律失常)发生率。结果:①睡眠呼吸指标状况,治疗12周后2组最长睡眠呼吸暂停时长及AHI较治疗前减少、SpO2较治疗前增高,且研究组最长睡眠呼吸暂停时长及AHI少于对照组、SpO2高于对照组(P<0.05);②临床疗效,研究组总有效率(94.44%)高于对照组(77.78%),P<0.05;③心脑血管事件,研究组心脑血管事件发生率(5.56%)低于对照组(22.22%),P<0.05。结论:联合采取低温等离子UPPP及持续正压通气治疗中重度OSAHS患者,可有效改善其睡眠呼吸暂停时长及睡眠呼吸紊乱指数等,利于提升疾病整体治疗效果,且可降低心脑血管事件发生风险,对改善预后效果具有一定积极意义。
