Comparative study on the short-term efficacy of bio-absorbable anchoring nails vs metallic anchoring nails for the treatment of TMJ anterior disc displacement without reduction

可吸收带线锚固钉与金属锚固钉治疗颞下颌关节盘不可复性前移位的短期疗效比较

Abstract:

Objective: To compare the short-term efficacy of bio-absorbable anchoring nails versus metallic anchoring nails for treatment of TMJ anterior disc displacement without reduction(ADDWoR). Methods: A total of 40 patients(52 sides) diagnosed as TMJ internal derangement(ID) who underwent modified temporomandibular joint disc anchor surgery in our hospital between Jan. 2019 and Dec. 2020 were divided into two groups. 20 patients were in metallic anchoring nails group (Group A, 27 sides) and 20 patients in bio-absorbable anchoring nails group (Group B, 25 sides). The scores for preoperative maximal interincisal opening (MIO), visual analogue scale (VAS) and magnetic resonance imaging (MRI) before surgery, and 3 months after surgery were compared within 2 groups. Results: The effective rate of MRI evaluation in group A was 96.29% (26 of 27 sides). The MIO was(38.74±3.23) mm. The VAS was 8.71±5.82. The effective rate of MRI evaluation in group B was 92.00% (25 of 27 sides). The MIO was (39.23±2.86) mm. The VAS was 8.71±5.82. There was no statistical difference between the two groups (P>0.05). Conclusion: The bio-absorbable anchoring nails for treatment of TMJ ADDWoR are effective and reliable. They can be adsorbed and have less interference to medical image examination. The long-term results still need further study.

目的:研究可吸收带线锚固钉与金属锚固钉应用于颞下颌关节盘锚固术治疗颞下颌关节盘不可复性前移位(anterior disc displacement without reduction, ADDWoR)的短期疗效比较。方法:选取2019年1月—2020年12月于我院行颞下颌关节盘锚固术的40例(52侧)患者,使用钛合金金属锚固钉(A组,27侧)和可吸收带线锚固钉(B组,25侧)的患者各为20例。比较2组术后磁共振(magnetic resonance imaging, MRI)数据以评价关节盘复位的有效率,以及术前、术后3个月随访时患者最大张口度(maximal interincisal opening, MIO)及视觉模拟量表(visual analogue scale,VAS)评分值的变化。结果:A组术后3个月时的MRI评价有效率为96.29%(26/27),MIO为(38.74±3.23) mm,VAS值为8.71±5.82,B组术后3个月时的MRI评价有效率为92.00%(23/25),MIO为(39.23±2.86) mm,VAS值为7.38±6.54,组间比较,差异无统计学意义(P>0.05)。结论:可吸收带线锚固钉应用于颞下颌关节盘锚固术治疗ADDWoR是可靠有效的,与金属锚固钉相比,其植入的锚固钉具有可吸收、对影像学检查干扰较小的优点,但其远期效果还有待于进一步研究。
