Abstract:

Objective: To evaluate the feasibility and clinical effect of the immediate implantation and restoration of multiple adjacent teeth in the aesthetic zone. Methods: From November 2015 to June 2018, 21 patients who received immediate implant placement because multiple adjacent anterior teeth couldn′t be retained, were recruited in this study in the department of implantology, Peking University School and Hospital of Stomatology. The patients were divided into maxilla group(10 cases) and mandible group(11 cases). All the patients received the immediate implantation and restoration protocol(T₀), and the permanent dentures were delivered 6 months after the implantation. At the observation point of one year after surgery(T₁), the survival rates of the implant and the prosthesis, marginal bone loss(MBL) were recorded. The pink esthetic score(PES) and Jemt papilla score were recorded as well at the time point of T₀ and T₁. Results: The survival rates of implant and prosthesis were 100% one year after surgery. For the maxilla group with 10 patients and 21 implants, PES score of T₀ was（12.5±2.8） and Jemt papilla score was （2.1±0.7）, of T₁ was （11.8±3.0） and （1.9±0.7）, respectively; MBL was （0.4±0.5） mm mesial and （0.5±0.6） mm distal of T₁. For the mandible group with 11 patients and 22 implants, PES score of T₀ was （8.7±3.1） and Jemt papilla score was （1.6±0.5）, of T₁ was （7.9±2.0） and （1.5±0.7）, respectively; MBL was （0.2±0.3 ） mm mesial and （0.3±0.5） mm distal of T₁. The differences of PES and Jemt score between the two groups at T₁ and T₀ points were not statistically significant（P>0.05）. Conclusion: During the observation period, the clinical result of the immediate implantation and restoration of multiple adjacent teeth in the aesthetic zone was stable and the preservation effect of interdental papilla was good.

目的： 探讨前牙区连续多牙即刻种植、即刻修复治疗的可行性及临床效果。方法： 选取2015年11月—2018年6月就诊于北京大学口腔医院种植科的相邻多个前牙无法保留而行即刻种植的21例患者。将患者分为上颌组（10例）和下颌组（11例），于即刻种植手术当天（T₀）行即刻修复，术后6个月行永久修复。术后1年（T₁）评价修复体、种植体存留率及种植体边缘骨吸收（marginal bone loss，MBL）情况；分别于T₀和T₁时对患者进行软组织美学（pink esthetic score，PES）评分及Jemt牙间乳头评分。结果： T₁时，2组修复体存留率为100%，种植体存留率为100%。上颌组：10例患者共植入21枚种植体，T₀时，PES评分为（12.5±2.8）分，Jemt牙间乳头评分为（2.1±0.7）分；T₁时，PES 评分为（11.8±3.0）分，Jemt牙间乳头评分为（1.9±0.7）分，种植体近中MBL为（0.4±0.5） mm，种植体远中MBL为（0.5±0.6） mm。下颌组: 11例患者共植入22枚种植体，T₀时，PES评分为（8.7±3.1）分，Jemt牙间乳头评分为（1.6±0.5）分；T₁时，PES评分为（7.9±2.0）分，Jemt牙间乳头评分为（1.5±0.7）分，种植体近中MBL为（0.2±0.3） mm，种植体远中MBL为（0.3±0.5） mm。分别比较两组T₁与T₀时的PES、Jemt牙间乳头评分，差异均无统计学意义（P>0.05）。结论： 观察期内，前牙区连续多牙即刻种植、即刻修复效果稳定，牙间乳头保存效果好。
