血凝块稳定化策略治疗干槽症的临床总结

Clinical summary of a blood clot stabilization strategy for the treatment of alveolar osteitis

摘要:

目的:

探讨基于血凝块稳定化理念的治疗策略在干槽症临床治疗中的应用效果,并评价其对疼痛缓解及创口愈合的影响。

方法:

收集2020年1月至2026年4月于中南大学湘雅医学院附属海口市人民医院口腔颌面外科就诊的30例(共33颗患牙)干槽症患者的临床资料。根据治疗方式将患者分为观察组(15例)和对照组(15例)。观察组采用血凝块稳定化治疗策略,在局部麻醉下彻底清创并建立新鲜出血创面,对出血不足者制备牙槽骨壁穿孔,采用自体浓缩生长因子/富血小板纤维蛋白(concentrated growth factor/platelet-rich fibrin,CGF/PRF)或自体静脉血重建血凝块,以胶原蛋白海绵覆盖并缝合固定;对照组常规清创后采用碘仿纱条填塞治疗。采用视觉模拟评分法(visual analogue scale,VAS)评价治疗前及治疗后24、48 h及7 d的疼痛变化,并比较2组临床疗效。

结果:

2组患者治疗后疼痛均逐渐缓解。治疗前,2组VAS评分差异无统计学意义(P>0.05);治疗后24、48 h及7 d,观察组VAS评分显著低于对照组(均P<0.001)。观察组7例(46.7%)于24 h内达到好转标准,其余8例(53.3%)于24~48 h达到好转标准;7 d随访时全部达到临床治愈。对照组疼痛缓解相对较慢,2例未达到好转标准;7 d时治愈率为46.7%。

结论:

基于血凝块稳定化理念的治疗策略在早期疼痛缓解和创口愈合方面显示出一定优势,为干槽症治疗提供了临床参考。

Abstract:

Objective:

To investigate the clinical efficacy of a blood clot stabilization-based therapeutic strategy for the treatment of alveolar osteitis and to evaluate its effects on pain relief and wound healing.

Methods:

Clinical data of 30 patients with alveolar osteitis (33 affected teeth) treated in the Department of Oral and Maxillofacial Surgery, Haikou People's Hospital Affiliated to Xiangya School of Medicine, Central South University, from January 2020 to April 2026 were analyzed. According to the treatment strategy, patients were divided into an observation group (blood clot stabilization strategy, n=15) and a control group (conventional iodoform gauze dressing, n=15). In the observation group, thorough debridement was performed under local anesthesia to establish a fresh bleeding socket. For sockets with insufficient bleeding, perforation of the alveolar bone wall was performed to induce bleeding. The blood clot was reconstructed using autologous concentrated growth factor/platelet-rich fibrin (CGF/PRF) or autologous venous blood, followed by coverage with a collagen sponge and fixation with sutures. The control group received conventional debridement followed by iodoform gauze dressing. Pain intensity was assessed using the visual analogue scale (VAS) before treatment and at 24, 48 h, and 7 d after treatment. Clinical outcomes were compared between the two groups.

Results:

Pain was gradually alleviated in both groups after treatment. There was no significant difference in pretreatment VAS scores between the two groups (P>0.05). However, the VAS scores in the observation group were significantly lower than those in the control group at 24, 48 h, and 7 d after treatment (all P<0.001). In the observation group, 7 patients (46.7%) met the criteria for clinical improvement within 24 h, and the remaining 8 patients (53.3%) met the criteria within 24-48 h. All patients achieved clinical healing at the 7-day follow-up. In the control group, pain relief occurred more slowly, and two patients failed to meet the criteria for clinical improvement. The clinical healing rate at 7 d was 46.7%.

Conclusion:

The blood clot stabilization-based therapeutic strategy demonstrated advantages in early pain relief and wound healing, providing a promising therapeutic option for the clinical management of alveolar osteitis.
