超声骨刀辅助“掀盖法”拔除左下颌埋伏牙1例报告及文献复习

Piezosurgery-assisted bone lid technique for removal of an impacted left mandibular tooth: A case report and literature review

本文报告1例采用超声骨刀辅助“掀盖法”拔除左下颌骨埋伏第一磨牙的病例,并结合文献探讨该术式的临床应用价值。术中采用超声骨刀制备骨盖,完成拔牙后将骨盖复位,以减少骨组织去除并维持术区骨性外形,在充分暴露术野的同时保护牙槽骨结构。术后即刻及术后3、6个月的连续随访结果显示,患者术后反应轻,肿胀及疼痛不明显,影像学检查示骨盖复位稳定,与周围骨组织结合良好,拔牙窝内可见新生骨形成。本病例提示超声骨刀辅助“掀盖法”可在充分暴露术野的同时保留骨组织完整性,为深部骨性埋伏牙的微创拔除提供一种可行的治疗方案。

Abstract:

This report presents a case of an impacted left mandibular first molar removed using a piezosurgery-assisted bone lid technique, and discusses the clinical application value of this surgical technique with reference to the literature. During surgery, a bone lid was created using a piezosurgery osteotome and repositioned after tooth extraction to minimize bone removal and preserve the osseous contour of the surgical site while providing adequate surgical access and protecting the alveolar bone. Follow-up examinations performed immediately after surgery and at 3 and 6 months postoperatively demonstrated mild postoperative reactions with minimal swelling and pain. Radiographic evaluation showed stable repositioning of the bone lid with satisfactory integration into the surrounding bone, and new bone formation was observed within the extraction socket. These findings suggest that the piezosurgery-assisted bone lid technique preserves the integrity of the alveolar bone while providing sufficient surgical exposure and may represent a feasible minimally invasive approach for the removal of deeply impacted teeth.
