应用超声骨刀拔除下颌第三磨牙临床效果的Meta分析

The Clinical Effect of Mandibular Third Molar Extraction with Piezosurgery: a Meta-analysis

摘要: 目的:采用 Meta 分析评价应用超声骨刀拔除下颌第三磨牙的临床效果。方法:计算机检索CBM、CNKI、VIP 数据库和万方数据库,同时对检索结果的参考文献进行检索,检索时间为建库至2019-03-01,搜集应用超声骨刀与单纯涡轮钻法拔除下颌阻生第三磨牙临床效果的随机对照试验文献,对检索结果进行独立质量评估和数据提取,采用RevMan 5.3软件进行Meta分析。结果:9篇文献纳入研究分析,包括实验组534 例,对照组529例。Meta分析表明应用或者联合应用超声骨刀与单纯涡轮钻法拔除下颌阻生第三磨牙相比,手术时间缩短(P<0.01),术后疼痛程度减轻(P<0.01),肿胀和张口受限的发生率降低(P<0.01)。结论:应用或联合应用超声骨刀法拔除下颌阻生第三磨牙的临床效果优于单纯涡轮钻方法。

Abstract: Objective: To evaluate the clinical effect of extraction of the mandibular third molar using piezosurgery. Methods: The database of CBM, CNKI, VIP, and Wanfang were searched up to March 1st, 2019. Literatures of randomized controlled trials (RCTs) comparing the clinical effects of piezosurgery and rotary instruments in extracting mandibular impacted teeth were collected. After data extraction and quality evaluation, a meta-analysis was carried out with Revman 5.3 software. Results: 9 clinical research papers were selected to analyse, including 534 cases in experimental group and 529 cases in control group. Compared with using rotary instruments, the operation time of tooth extraction was shorter (P<0.01), using or combining with piezosurgery, with lower scores of postoperative pain(P<0.01), and the lower incidence of complications such as facial swelling and limited mouth opening(P<0.01). Conclusion: Mandibular impacted third molar extraction using or combining with piezosurgery is clinically superior to using rotary instruments alone.
