口腔肿瘤患者心理压力与体内相关因素变化的初探

Pilot Study on Assessment of Depression and Stress Level among Oral Tumor Patients in Relation to Variation of Relevant Material Basis

摘要: 目的: 认识口腔肿瘤患者精神因素变化规律,探索其相关物质基础。方法: 我院2011-12—2012-08间确诊的口腔恶性肿瘤患者40例,良性肿瘤患者35例,应用流行病学调查中心抑郁量表(CES-D)和压力知觉量表(PSS)测评每位受试者抑郁以及心理压力状态。于入院次日上午9:00抽取受试者的全血,切取新鲜术后肿瘤组织,分离提取血清,匀浆肿瘤组织,采用高效液相色谱-质谱(HPLC-MS/MS)分析血清中儿茶酚胺以及糖皮质激素含量。结果: 口腔恶性肿瘤组CES-D总分得分比口腔良性肿瘤组高,抑郁程度更重,差异有统计学意义(P<0.05)。口腔恶性肿瘤组PSS总分得分与口腔良性肿瘤组之间差异没有统计学意义(P>0.05)。血清中肾上腺素、去甲肾上腺素、氢化可的松及可的松的检测结果,口腔恶性肿瘤组与口腔良性肿瘤组差异有统计学意义(P<0.001);对肿瘤组织的物质分析提示,恶性肿瘤组织中肾上腺素和去甲肾上腺素含量均高于良性肿瘤组织,差异没有统计学意义。结论: 口腔肿瘤患者存在明显的精神心理因素变化,同时伴有机体内相关物质变化,影响着恶性肿瘤的发生发展,对进一步研究恶性肿瘤疾病进程的相关机制及探索干预方法有一定价值。

Abstract: Objective: To investigate the difference of mental health status in oral tumor patients, and explore the difference of relevant material basis. Methods: 40 patients with oral cancer and 35 patients with oral benign tumor who were diagnosed in the West China Stomatology Hospital  between Dec 2011 and Aug 2012 were assessed with Center for Epidemiological Studies Depression Scale (CES-D) and Perceived Stress Scale (PSS). Participants were stuck with needle used to take a blood sample on second admission day and got fresh tumor tissue about 0.5 g. A method was developed for the determination of catecholamine and glucocorticoid in serum using liquid chromatography-tandem mass spectrometry (HPLC-MS/MS). Results: The scores of CES-D between oral cancer group and oral benign tumor group showed  statistical significance(P<0.05). The difference of the scores of PSS were not statistically significant. However, the content of epinephrine, norepinephrine, cortisone hydrocortisone in serum using HPLC-MS/MS manifested remarkable difference (P<0.001). The content of epinephrine, norepinephrine in tumor tissue using HPLC-MS/MS showed  no statistical difference (P>0.05). Conclusion: Psychiatric factors do not play as simple factors effect on patients, it may be deep foundation in the tumorigenesis and developing through small molecules like hormone. Further studies are needed.
