Caspase-3及p53预测口腔鳞癌动脉灌注化疗敏感性的观察

The Prediction Value of Caspase-3 and P53  in the Chemotherapy for Oral Squamous Cell Carcinoma

摘要: 目的:观察Caspase-3及p53预测口腔鳞癌行动脉灌注DF方案(顺铂+5-氟尿嘧啶)化疗疗效的准确性。方法:58例Caspase-3表达阳性和(或)p53表达阴性口腔鳞癌患者,行颞浅动脉插管、DF方案持续灌注化疗2周期后手术治疗。观察化疗前、后,原发肿瘤灶大小及病理学变化,分析Caspase-3及p53表达差异与化疗疗效的关系。结果:Caspase-3表达阳性同时p53表达阴性,对口腔鳞癌动脉灌注化疗完全缓解的预测准确性达93.55%,在病理上预测的准确性达90.32%。结论:应用Caspase-3和p53预测口腔鳞癌动脉灌注DF方案化疗敏感性,简便可靠,值得推广。

Abstract: Objective: To evaluate the expression of Caspase-3 and P53 in the tumor tissue and their prediction value for chemotherapy of oral squamous cell carcinoma (OSCC). Methods: 58 patients who had positive Caspase-3 expression and/or  negative p53 expression in OSCC were selected, and treated by two courses of continuous chemotherapies with Cisplatin and 5-Fu through intra-arterial infusion, and then underwent surgical dissections. Changes in tumor size  and clinical pathology were observed. The correlations between the expression difference of Caspase-3 and p53 and the degree of tumor regression were retrospectively analyzed. Results: For the patients with Caspase-3 positive expression/p53 negative expression, the predictive sensitiveness accuracy of complete response to infusion chemotherapy and the predictive accuracy of carcinoma structural damage reached 93.55% and 90.32% respectively. Conclusion: The combined analysis of Caspase-3 and p53 may show positive prediction value in chemotherapy of OSCC.
