摘要： 目的：研究大型颌骨囊肿开窗减压术联合囊肿塞治疗的临床应用。方法：对 35 例大型颌骨囊肿患者进行开窗减压术，去除囊内容物，保留囊壁，制作囊肿塞，保持囊内清洁。观察术后囊肿大小、囊壁与邻近结构及周围骨质变化情况，以评价该手术的临床效果。结果：术后随访 1～18个月，32例患者治疗效果明显，有效率高达92%。观察期内，颌骨骨皮质再生良好，CT示囊腔周围有放射状骨小梁结构，18个月时3例疗效差者行II期刮治术，效果良好，无复发。结论：开窗减压术联合囊肿塞治疗大型颌骨囊肿，具有操作简单、组织损伤小、能最大限度保存颌骨形态和生理功能、术后并发症少等优点，值得推广。

Abstract:  Objective: To evaluate  the clinical effects of marsupialization with space obliteration in the treatment of large cysts of the jaws. Methods: 35 cases of patients with large jaw cysts were treated by cutting an adequate window in the outer wall of the cyst through which the contents of the cyst were evacuated, but the remaining part of the membrane was not removed, making the cyst plug for space obliteration, keeping the cystic space clean. After operation, the variations size of the cyst, the cystic wall, the adjacent structure, and the surrounding bone were observed and evaluated. Results: After 1-18 months follow up, 32 cases (92%) achieved satisfactory results. Cystic cavity gradually diminish in size, natural repair process showed to restore the original outline and structure of the involved bone, 3 patients failed with the treatment and a secondary enucleation of the remaining membrane  were performed. Conclusion: Decompression and cyst plug in the treatment of large jaw cyst, which has the advantages of simple operation, little injury and preserve the morphology and physiological function of jaw, less postoperative complications, is worthy of promotion.
