间接数字化印模3D打印塞治器在下颌骨囊肿开窗减压治疗中的应用

Application of 3D Printing Plug Made by Indirect Digital Impression in the treatment of Mandible Cystic Lesions with Window Decompression

摘要: 目的:探究下颌骨囊肿开窗减压术后,利用间接数字化印模结合SLA原理3D打印塞治器,用于维护开窗囊腔的可行性。方法:选取下颌骨囊性病变患者9名,在行开窗减压术后4~7 d,采用直接法制取下颌骨囊腔蜡型。3 shape Trios激光扫描仪扫描蜡型获取数字印模,完成囊腔蜡型的三维重建,以德国BEGO Varseo Wax树脂为材料,通过SLA原理的3D打印机完成颌骨囊肿塞治器的制作。结果:采用间接数字化印模技术结合3D打印技术完成的9个颌骨囊肿的囊腔塞治器,无卡环及基托装置,纤巧、表面光滑,塞治器与开窗创口边缘密合,具有一定固位力,可阻挡囊腔窗口过早愈合,对囊液有引流作用。结论:通过SLA原理3D打印完成颌骨囊肿塞治器,制作方式简便高效,成品质量较好。适用于开窗口位于磨牙后区且术后张口受限无法取模的患者,以及开窗造口位于前庭沟区的患者。

Abstract: Objective: To explore the feasibility of 3D printing plug made by indirect digital impression, in the treatment of mandible cystic lesions with device marsupialization. Methods: Nine patients with mandibular cystic lesions were selected and performed 4-7 days after decompression. Firstly,we used the direct method to obtain mandibular cysts wax type, 3 shape Trios laser scanner scanned wax type to obtain digital impression, and completed the wax type of three-dimensional reconstruction model. Secondly, the individual cyst plug model was fabricated with BEGO Varseo Wax resin based on the technique of SLA. Results: Nine patients with mandibular cystic lesions were selected after decompression,we used indirect digital impression combined with 3D printing to complete the cyst plug. It had no clasp and base device, but slim and smooth surface. The cyst plug and the soft tissue had good marginal adaptation, it had a certain retention force, and could delay the soft tissue to close the cystic cavity, and cyst content could be drained out. Conclusion: Based on digital impression and 3D reconstruction, firstly, the digital model was made with wax type by using the 3 shape scanner. Secondly, a cyst plug model was finished based on the 3D model reconstruction. Thirdly, the individual cyst plug model was fabricated based on the technique of SLA. The fabrication of individual cyst plug with SLA was feasible. The whole progress was simple and efficient, and the products had good quality.
