颌骨血外渗性囊肿24例回顾性研究

Extravasation Cysts of the Jaw: A Retrospective Study of 24 Cases

摘要:

目的: 总结颌骨血外渗性囊肿(extravasation cyst)的临床、手术、影像学特点,以及其治疗策略与预后。方法: 收集2011年7月—2019年11月于南京大学医学院附属口腔医院就诊的颌骨血外渗性囊肿患者的病例资料,回顾性分析其临床、手术、影像学特点及随访情况。结果: 24例患者纳入本研究,其中男性11例,女性13例,囊肿均发生于下颌骨。20例患者无自觉症状,4例伴疼痛、颌面部膨隆或牙齿松动。20例无症状患者接受刮治术,4例有症状患者接受开窗减压术。病理检查均未见上皮衬里。13例有复查记录的患者中,7例完全愈合,6例颌骨骨腔内有新骨形成。结论: 颌骨血外渗性囊肿好发于下颌骨,大多无自觉症状,病理检查无上皮衬里。开窗减压术或刮治术是治疗颌骨血外渗性囊肿的有效方法。

Abstract:

Objective: To summarize the clinical, imaging and surgical characteristics of jaw extravasation cyst as well as treatment strategy and prognosis. Methods: Patients diagnosed as extravasation cyst of the jaw in Nanjing Stomatological Hospital from July 2011 to November 2019 were included in this retrospective study, and their clinical, imaging, surgical characteristics and follow-up information were collected. Results: 24 patients were included in this study, including 11 males and 13 females. All the cyst lesions were in the mandible. 20 cases were asymptomatic and 4 cases developed symptoms of pain, maxillofacial bulging or tooth mobility. 20 cases received cyst curettage and 4 cases received fenestration. No epithelial lining were found in histopathological examination of the specimen. 13 cases had follow-up visits for radiological examinations, and 7 cases of them exhibited complete bone healing while 6 cases showed new bone formation. Conclusion: Jaw extravasation cyst usually occurs in the mandible with no symptoms, and shows no epithelial lining in histopathological examination. Fenestration and cyst curettage are both effective for the treatment of jaw extravasation cyst.
