炎症指标鉴别口腔颌面部间隙感染致病菌类型作用的分析

摘要:

目的:探讨炎症指标在鉴别口腔颌面部间隙感染致病菌类型的作用。方法:对116名本院口腔颌面外科感染患者的临床资料进行回顾性分析,研究相关炎症指标在革兰阳性菌和革兰阴性菌两组间的差异。结果:在116名患者中,细菌培养结果为革兰阳性菌的有56例,革兰阴性菌有16例。革兰阳性菌组降钙素原(procalcitonin,PCT)高于革兰阴性菌组,差异具有统计学意义(P<0.05)。在细菌培养结果为阳性的患者中,排除高血压、糖尿病患者,剩余革兰阳性菌者有30例,革兰阴性菌者有10例。革兰阴性菌组中白细胞数、中性粒细胞数均高于革兰阳性菌组,差异具有统计学意义(P<0.05)。结论:在口腔颌面部间隙感染的患者中,革兰阳性菌感染,PCT值较高,尤其见于并存基础疾病的情况下;革兰阴性菌感染,在无基础疾病情况下,白细胞及中性粒细胞数较高。

Abstract:

Objective: To investigate the role of inflammatory markers in differentiating pathogenic bacteria of oral and maxillofacial infections. Methods: The clinical features of 116 patients with oral and maxillofacial space infection were collected and analyzed retrospectively. Analyze the differences of procalcitonin(PCT) and other inflammatory markers in oral and maxillofacial space infection. Results: Gram positive bacteria infection revealed in 56 patients and gram negative bacteria in  16 patients. PCT level in G+ patients was significantly higher than in G- patients (P<0.05).  The number of leukocytes and neutrophil counts in G-  group were higher than those in G+ group, the difference was statistically significant (P<0.05). Moreover, there was no significant difference between the two groups regarding other inflammatory markers. Conclusion: In the oral and maxillofacial infectious with G+ bacteria,  demonstrated a higher PCT value, especially in those patients combined with a basic chronic diseases situation. In G-  infection patients, often showed a higher white blood cells and neutrophils counts.
