双膦酸盐相关性颌骨坏死微血管密度及VEGF的水平变化

Changes of Microvascular Density and Vascular Endothelial Growth Factor Level in Bisphosphonate-related Osteonecrosis of the Jaws

摘要: 目的:探讨双膦酸盐相关性颌骨坏死病损组织中血管内皮生长因子(VEGF)的表达,微血管生成及其与临床分期的关系。方法:采用S-P免疫组织化学染色法检测24例双膦酸盐相关性颌骨坏死(观察组),17例颌骨隆突(对照组)组织中VEGF的表达及微血管密度(MVD)。结果:观察组的VEGF表达阳性率为25%(6/24),MVD为(2.04±0.81) mm2,显著低于对照组的VEGF阳性表达率(64.7%)和MVD值(7.06±1.71) mm2(P<0.05),且VEGF与MVD值呈正相关(P<0.05)。观察组按美国口腔颌面外科医师协会确立的标准进行分期,0~Ⅰ期14例,Ⅱ~Ⅲ期10例,二组在VEGF的阳性表达率、MVD无显著性差异(P>0.05),与临床分期不相关。结论:双膦酸盐相关性颌骨坏死病损区微血管生成受到抑制,且与血管内皮生长因子低度表达相关。

Abstract: Objective: To investigate the expression of vascular endothelial growth factor (VEGF) and micro-vascular angiogenesis in bisphosphonate-associated osteonecrosis of the jaws(BRONJ), and their relationships with clinical stages of BRONJ. Methods: S-P immunohistochemical staining was used to detect the expression of VEGF and microvascular density (MVD) in the lesions of 24 patients with bisphosphonate-associated jaw necrosis (observation group) and 17 patients with jaw juga (control group). Results: The positive rate of VEGF expression in observation group (25%) was significantly lower than that in control group (64.7%), as well as the MVD (2.04±0.81/mm2 vs 7.06±1.71/mm2) (P<0.05), and the level of VEGF was positively correlated with the MVD(P<0.05). Observation group was divided into two groups according to the staging standard of American association of oral and maxillofacial surgeons, one group included 14 cases of stage 0~Ⅰand another group included 10 cases of stage Ⅱ~Ⅲ. There was no significant difference between these two groups in the positive expression rate of VEGF and MVD(P>0.05). Conclusion: Micro-vascular angiogenesis in the lesions of bisphosphonate-associated jaw bone necrosis is inhibited, which may be correlated with low expression of vascular endothelial growth factor.
