HIV negative polycystic lymphoepithelial cyst of parotid gland: A case report and literature review

摘要:

腮腺淋巴上皮囊肿(lymphoepithelial cyst,LEC)在临床上较为罕见且容易误诊,需与多种腮腺囊性病变鉴别。LEC被认为是人类免疫缺陷病毒(human immunodeficiency virus,HIV)阳性患者腮腺肿大的最常见病因,而在HIV阴性的腮腺肿物中占比极低。HIV阴性的LEC患者多表现为单侧腮腺单囊性病变,而腮腺区双侧或多囊性LEC则绝大多数见于HIV感染者。目前,LEC有多种治疗方式,结合文献分析,我们认为手术切除为LEC的首选治疗方式,硬化治疗及抗逆转录治疗对于HIV阳性者有较大意义,无治疗意愿者可保守观察。现报道1例我科收治的HIV阴性单侧多囊性腮腺LEC患者,临床上极为罕见。

Abstract:

Lymphoepithelial cyst (LEC) of the parotid gland is a rare lesion and easily misdiagnosed in clinic, it should be differentiated with a variety of parotid gland cystic diseases. LEC is considered to be the most common cause of parotid gland enlargement in human immunodeficiency virus (HIV)-positive patients, while its proportion is extremely low in HIV-negative parotid neoplasms. HIV-negative LEC patients are most likely to present as unilateral and unilocular parotid gland lesion, while bilateral or polycystic LEC of parotid gland is mostly seen in HIV-infected patients. Currently, there are a variety of treatment methods for LEC. With the literature review, we believe that surgical resection is the preferred treatment method for LEC, sclerotherapy and antiretroviral therapy are of great significance for HIV-positive patients, while conservative observation can be taken for those without treatment intentions. Recently, a HIV-negative patient with unilateral polycystic parotid LEC was admitted to our department, which is extremely rare in clinic and the report is as follows.
