摘要: 目的:利用锥形束CT(cone-beam computed tomography,CBCT)综合评估临床中双尖牙区采用即刻种植和引导骨组织再生技术(guided bone regeneration,GBR)进行位点保存的效果。方法:选择双尖牙需拔除并行种植修复的患者65例90颗牙,随机分为即刻种植GBR组(即刻种植+引导骨组织再生)、单纯GBR组(拔牙创GBR技术处理+延期种植)和空白对照组(拔牙创常规处理+延期种植)。拔牙前及拔牙后1、3、6个月分别行CBCT扫描,测量牙槽骨高度和宽度,并用Simplant软件测量位点骨密度,比较各时间点3组间各指标差异,评价位点保存的效果。结果:各时间点骨高度丧失量、骨宽度丧失量及骨密度变化值相比,即刻种植GBR组变化最小,单纯GBR处理组次之,空白对照组最大,每2组间差异均有统计学意义(P<0.05)。结论:双尖牙区应用即刻种植联合GBR处理,可有效进行种植位点保存,并能减少患者就诊次数,简化种植修复程序,适合在临床推广应用。

Abstract: Objective: The purpose of the present study was to evaluate the clinical site preservation effected by using immediate implanting technology in the bicuspid region. Methods: 65 patients (90 teeth) received informed consents with removal of bicuspid teeth were randomly divided into 3 groups: immediate implanting + GBR(guided bone regeneration) group (30 teeth), GBR alone group (30 teeth), and the control group (30 teeth). CBCT scanning examination was performed before and 1-, 3-, and 6- month after tooth extraction, to evaluate the alveolar bone height, width, and bone mineral density with the help of Simplant software. Results: The changes of bone height,width, and bone density at each timepoint of "immediate implanting+GBR" group showed the smallest, and there was a significant difference among the 3 groups (P<0.05). Conclusion: "Immediate implanting+GBR" can effectively achieve the socket and ridge preservation in the bicuspid region. It also can reduce the time length of treatment, simplify the process of restoration. Therefore, this method is suitable for clinical popularization and application.
