3D打印在第三磨牙自体移植中的临床研究

摘要:

目的: 探讨锥形束CT(cone-beam computed tomography, CBCT)联合3D打印制作的个性化牙模型在上、下颌第三磨牙移植修复缺失磨牙病例中的临床疗效。方法: 收集2017年5月—2019年4月在温州医科大学附属口腔医院上、下颌第一或第二磨牙缺失并需拔除第三磨牙的46例病例(52颗牙)。按照手术方式分组,对照组(25例,28颗牙)采用传统移植方案,观察组(21例,24颗牙)应用CBCT术前分析结合3D打印模型进行术前牙槽窝预备后即刻移植。术后1年复查,拍摄X线片,对患者满意度、移植牙松动情况、牙周探诊深度、牙根吸收情况,以及疗效进行统计。结果: 观察组患者满意度明显高于对照组,差异具有统计学意义(P<0.05)。观察组移植牙松动度分级、牙周探诊深度分级、牙根吸收情况分级明显低于对照组,差异具有统计学意义(P<0.05)。观察组移植牙疗效评价明显高于对照组,差异具有统计学意义(P<0.05)。结论: 在临床自体牙移植病例中,3D打印牙模型的应用有效提高了移植牙的成功率,可以作为一种新的治疗方案应用于临床牙列缺损的病例。

Abstract:

Objective: To explore the clinical efficacy of cone-beam computed tomography(CBCT) combined with 3D printing on a personalized tooth model in the case of third molar autotransplantation. Methods: From May 2017 to April 2019, 46 cases(52 teeth) with missing first or second molars, and a third molar extraction were collected in the Affiliated Stomatological Hospital of Wenzhou Medical University. Subjects were divided into 2 groups according to the surgical method. The control group(25 cases, 28 teeth) used the traditional transplantation plan, and the study group (21 cases, 24 teeth) used CBCT preoperative analysis combined with 3D printed models for preoperative alveolar socket preparation and immediate transplantation. One year after the operation, X-rays examination were taken, and statistical analysis were measured based on patient satisfaction, transplanted tooth mobility, periodontal probing depth, root resorption and curative effect. Results: The satisfaction degree of patients in the study group was significantly higher than that in the control group, and the difference was statistically significant(P<0.05). The grading of transplanted tooth mobility, periodontal probing depth, and root resorption of the study group were significantly lower than those of the control group, and the differences were statistically significant(P<0.05). The evaluation of curative effect of transplantation in the study group was significantly higher than that in the control group, and the difference was statistically significant (P<0.05). Conclusion: The application of 3D printed tooth model effectively improves the success rate of tooth transplantation in clinical autologous tooth transplantation cases. It can be used as a new treatment program in clinical cases of dentition defect.
