糖尿病患者种植术后早期种植体周围炎龈沟液中相关因子表达

Expression of Cytokines Levels in the Peri-implant Gingival Crevicular Fluid of Diabetic Patients

摘要:

目的:探讨糖尿病患者种植体周围龈沟液量及龈沟液中IL-1β、IL-6IL-8IL-10TFN-α和IFN-γ水平的表达。方法:选择2016-032018-03在河北省石家庄市赵县妇幼保健院口腔科种植科进行ITI种植体植入且全瓷冠修复的糖尿病患者30例。另选30例同期血糖正常的种植牙患者作为对照组。统计糖尿病患者组和正常患者组种植体临床指标(牙周探诊、龈沟出血指数)、种植体龈沟液量及种植体龈沟液中IL-1β、 IL-6IL-8IL-10TFN-α和IFN-γ水平的表达。结果:糖尿病患者组种植体临床指标(牙周探诊、龈沟出血指数)与正常患者组比较,差异无统计学意义(P0.05),而糖尿病患者组种植体龈沟液量低于正常患者组(P0.05)。与正常患者组相比,糖尿病患者组种植体龈沟液中表达的IL-1β、IL-8TFN-α、IFN-γ明显升高(P0.05),IL-10表达量明显降低(P0.05),而IL-8表达量差异无统计学意义(P0.05)。结论:糖尿病患者组种植体龈沟液量少,其高表达IL-1β、IL-8TFN-α、IFN-γ,而低表达IL-10

Abstract:

Objective: To investigate the peri-implant gingival crevicular fluid volume and the express ion levels of IL-1β, IL-6, IL-8, IL-10, TFN-α and IFN-γ in the gingival crevicular fluid of diabetic mellitus (DM) patients. Methods: Thirty patients with DM  who underwent implantation of ITI implants and all-ceramic restorations between March 2016 to March 2018 were selected. Thirty patients in normal blood sugar level with implanted teeth were also selected as control. Statistical analysis was conducted between clinical indications (periodontal probing, gingival crevicular bleeding index), amount of gingival crevicular fluid in implants, and the expressions of IL-1β, IL-6, IL-8, and IL-10, TFN-α and IFN-γ in implant gingival crevicular fluid. Results: There was no significant difference in clinical indications  (periodontal probing, gingival sulcus bleeding index) between the diabetic group and the normal patient group (P>0.05), while the gingival fluid volume of the diabetic patient group was lower than that of the normal patient group (P<0.05). Compared with the normal patient group, IL-1β, IL-8, TFN-α, and IFN-γ expression  in the gingival crevicular fluid of the implanted diabetic group were significantly higher (P<0.05), and the expression of IL-10 was significantly lower (P<0.05), but there was no significant difference in the expression of IL-8 (P>0.05). Conclusions: The amount of peri-implant gingival crevicular fluid in the diabetic patients was less than the control patients, but highly expressed IL-1β, IL-8, TFN-α, IFN-γ and IL-10 cytokines levels.
