微创技术在拔除下颌低位埋伏阻生牙中的应用

Minimally Invasive Technique in Odontectomy of Low Level Impacted Wisdom Teeth of the Mandible

摘要: 目的:探讨在拔除下颌低位埋伏阻生智齿的过程中采用微创技术的临床应用效果。方法:随机选取2014-06—2016-03间在我院就诊的下颌低位埋伏阻生牙拔除患者100例,其中分别包括微创技术拔除患者和传统凿骨劈冠拔除患者各50例,并分别将其纳入本次探究的实验组和对照组,比较和分析两组治疗的效果。结果:对照组患者的手术时间及术后恢复时间分别为(27.86±11.20) min、(15.3±3.70) d,手术时间及术后恢复所需时间均明显高于实验组的(18.67±8.12) min、(9.60±2.90) d;对照组患者术后重度肿胀及重度疼痛的发生率分别为68%和64%,显著高于实验组发生率(30%、38%);对照组患者发生口角损伤等并发症的发生率合计为72%显著高于实验组(10%)。上述所有的组间差异有统计学意义(P<0.05)。结论:微创手术可以缩短手术时间,降低手术创伤性,改善手术预后,临床应用效果显著。

Abstract:  Objective: To evaluate and discuss the clinical value of minimally invasive technique in removal of low level impacted wisdom teeth of the mandible. Methods: From June 2013-March 2016, 100 patients were randomly selected. Odontectomy performed with minimally invasive technique (trial group, n=50), odontectomy performed with conventional bone-excision, tooth -section technique (control group, n=50), the results were compared and analyzed. Results: The operation time length and postoperative recovery time in control group were (27.86±11.2) min,  and (15.3±3.7) d respectively, they were significantly higher than  trail group[(18.67±8.12) min and (9.6±2.9) d]. Postoperative severe swelling and severe pain incidence rate in control group, were 68.00% and 64.00%, which were significantly higher than that of trail group(30.00% and 38.00% respectively). Total complications as spat injury incidence rate in control group was 72.00%, while it in trail group was 10%. The difference between the two groups was statistically significant (P<0.05). Conclusion: This paper demonstrates multiple clinical values of minimally invasive surgery in odontectomy of impacted mandibular wisdom teeth.
