开窗减压引流术在下颌骨囊性病变治疗中的临床观察

摘要:

目的:分析与评估开窗减压引流术在下颌骨囊性病变治疗中的临床意义。方法:回顾101例下颌骨囊性病变行开窗减压引流术的病例资料,并进行疗效对比分析。其中,牙源性角化囊性瘤(keratocystic odontogenic tumor,KCOT)53 例,含牙囊肿(dentigerous cysts, DC)12例,成釉细胞瘤(ameloblastoma, AB)36例。结果:101例患者术后均有不同程度的囊腔面积缩小。以单囊型角化囊性瘤及成釉细胞瘤的囊腔面积缩小最为明显;实性型或多囊型成釉细胞瘤缩小不明显,预后较差。结论:开窗减压引流术对于下颌骨巨大囊性病变及位于重要组织结构周围的病变是一种简单有效的治疗方法,疗效显著,值得临床进一步推广与应用。但术前需进行病例的选择,单囊型囊性病变为首选,多囊及实性型囊性病变的效果欠佳。

Abstract:

Objective: To analyze and evaluate the clinical significance of decompression by open drainage in treating mandibular cystic lesions. Methods: A total of 101 cases of mandibular cystic lesions which received decompression by open drainage were retrospectively analyzed and the clinical outcomes were used to evaluate the significance of treatment. 101 cases including 53 cases of keratocystic odontogenic tumor(KCOT), 12 cases of dentigerous cysts(DC), 36 cases of ameloblastoma(AB). Results: All demonstrated a considerable degree of reduction in the size of cystic cavity. Unicystic KCOT and AB showed the most obvious reduction of the cystic cavity. Solid or multicystic AB showed no obvious reduction, and the prognosis was poor. Conclusion: Decompression by open drainage is a simple and effective method in treating mandibular huge cystic lesions and the lesions located around the important structures. It has a significant effect and is worthy of further clinical promotion and application. But case selection of indication is important before surgery. Unicystic cases are the first choice and solid or multicystic cases are less effective.
