三维CT引导下经卵圆孔穿刺射频治疗三叉神经痛

Radiofrequency Thermocoagulation for Trigeminal Neuralgia through 3D-CT-guided Ovale Foramen

摘要: 目的:探讨三维CT引导下经卵圆孔穿刺射频治疗三叉神经痛的临床疗效。方法:随机选取我院2013—2015年收治的三叉神经痛患者38例,行三维CT引导卵圆孔穿刺射频术治疗,对其临床疗效和术后并发症进行分析,并进行6个月的随访。结果:38例患者行三维CT引导卵圆孔穿刺射频治疗三叉神经痛手术均成功。术后第1天有效率达78.95%,12例患者完全缓解,有1例未缓解。术后1个月与术后6个月的有效率分别为94.74%和97.37%,二者之间差异不具有统计学意义(P>0.05),且均显著高于术后第1天和第7天,差异均具有统计学意义(P<0.05)。结论:三维CT引导下经卵圆孔穿刺射频治疗三叉神经痛可提高穿刺成功率,减少并发症,选择性毁损痛觉纤维有助于提高患者生活质量,值得临床推广使用。

Abstract: Objective: To investigate the clinical efficacy of 3D-CT-guided ovale foramen locations in radiofrequency thermocoagulation for trigeminal neuralgia. Methods: 38 cases with trigeminal neuralgia in our hospital from 2012 to 2015 were randomly selected for the study and treated with 3D-CT-guided ovale foramen locations in radiofrequency thermocoagulation. The clinical efficacy and postoperative complications were analyzed and the patients were followed up for 6 months. Results: The operations for 38 patients were successful. The effective rate of the patients on the first day after operation was 78.95%. 12 cases of patients  revealed a complete remission, except one patient with no remission. The effective rate increased to 94.74% one month after therapy, and the effective rate was 97.37% at six month after therapy, without statistical difference (P>0.05) between the effective rates of the two time points. Both the effective rate were significantly higher than the first day and 7 days after therapy, and the difference were statistically significant (P<0.05). Conclusion: The treatment of 3D-CT-guided ovale foramen locations in radiofrequency thermocoagulation for trigeminal neuralgia, can improve the success rate of operation, reduce complications, and is worthy of clinical promotion.
