MTH1在口腔鳞癌中的表达及其对预后的影响

The expression of MTH1 in oral squamous cell carcinoma and it’s effect on prognosis

摘要:

目的:研究MutT 同源酶 1(MTH1)在口腔鳞状细胞癌(简称口腔鳞癌)组织中的表达及表达情况对患者预后的影响。方法:应用免疫组织化学方法对62例口腔鳞癌患者的石蜡切片进行检测,并对患者的预后情况进行随访,应用Kaplan-Meier法进行生存率分析,应用Cox回归进行多因素分析,应用蛋白质免疫印记实验对29例口腔鳞癌患者的新鲜组织进行检测。结果:免疫组化检测与蛋白质免疫印迹实验发现肿瘤组织中MTH1的的表达水平均高于正常组织,且伴随着口腔鳞癌组织的分化程度降低,MTH1的表达水平升高;MTH1的表达程度与患者的性别、年龄、吸烟史、饮酒史、患病部位均无明显关系(P>0.05),而与肿瘤浸润深度(P<0.000 1)、淋巴结转移情况(P=0.000 8)及肿瘤分化程度(P<0.000 1)有明显关系。MTH1高表达组患者5年总生存率及肿瘤特异性生存率均低于MTH1低表达组患者(P<0.000 1),且通过单因素及多因素Cox回归分析发现,肿瘤浸润深度(P=0.002 0),淋巴结转移情况(P=0.024 8),肿瘤分化程度(P=0.025 1)及MTH1表达情况(P≤0.000 1)均与患者的预后情况有密切联系,其中MTH1在口腔鳞癌中的表达可作为独立的影响因素(P=0.011)来判断患者的预后情况。结论:MTH1的表达情况从一定程度上反映了口腔鳞癌疾病的严重程度,并提示口腔鳞癌患者的预后情况,MTH1有望成为诊疗中对口腔鳞癌患者预后进行评估的新指标。

Abstract:

Objective: To investigate the expression level of MutT homolog 1 (MTH1) in oral squamous cell carcinoma(OSCC) and its impact on patients’ prognosis. Methods: The paraffin-embeded cancer tissue sections of 62 patients with oral squamous cell carcinoma were analyzed by immunohistochemistry and the prognosis of all patients were followed-up. Kaplan-Meier analysis was used to assess the survival rate, and Cox regression was used for multivariate analysis. The fresh tissue of 29 patients with oral squamous cell carcinoma was collected for Western blotting. Results: Immunohistochemistry and Western blotting analysis showed that the expression level of MTH1 in tumor tissues was higher than that in normal tissues, and the higher expression level of MTH1 was related to the lower differentiation level of OSCC cells. The expression level of MTH1 has no obvious relationship with gender, age, smoking history, drinking history, and the position of primary tumor (all P>0.05). Meanwhile, it was significantly correlated with tumor invasion depth (P<0.000 1), lymph node metastasis (P=0.000 8) and tumor cell differentiation (P<0.000 1). The five-year overall survival rate and tumor-specific survival rate of patients with high MTH1 expression were lower than those with low MTH1 expression(P<0.000 1). And by the univariate and multivariate Cox regression analysis, we found that the depth of tumor invasion (P=0.002 0), lymph node metastasis (P=0.024 8), tumor differentiation (P=0.025 1) and MTH1 expression (P≤0.000 1) were closely related to the prognosis of OSCC patients, among them, MTH1 can be used as an independent factor (P=0.011) to predict the prognosis of patients with oral squamous cell carcinoma. Conclusion: The expression of MTH1 reflects the severity of oral squamous cell carcinoma to certain extent, and suggests the prognosis of OSCC patients. MTH1 can be expected as a new predictor for the prognosis of patients with OSCC.
